Malignant transformation of benign hidroacanthoma simplex

Summary

This case study reports malignant hidroacanthoma simplex in a 75-year-old man. Immunohistochemical analysis suggests tumor origin from intraepidermal duct outer cells, correlating with malignant transformation.

Background:

Hidroacanthoma simplex is a rare skin adnexal tumor.
Malignant transformation of benign hidroacanthoma simplex is exceptionally uncommon.

Observation:

A 75-year-old male presented with a lesion on the anterior right ankle.
Initial biopsy in 1987 indicated benign hidroacanthoma simplex.
A subsequent biopsy in 1991 revealed malignant transformation.

Findings:

Immunohistochemical studies were performed on both specimens.
Results suggest the tumor originates from the outer cells of intraepidermal sweat ducts.
Proliferating cell nuclear antigen (PCNA) labelling index and argyrophil nucleolar organizer regions (AgNORs) were compared and correlated with malignant progression.
