Background:

  • Progressive Autonomic Failure (PAF) is a neurodegenerative disorder affecting the autonomic nervous system.
  • Clinical presentation often suggests postganglionic sympathetic dysfunction.

Observation:

  • Two cases of pure PAF were analyzed.
  • A postmortem examination of one case revealed neuropathology similar to Parkinson's disease.

Findings:

  • Significant neuronal loss was observed in the substantia nigra, nucleus ceruleus, and spinal cord's intermediolateral column.
  • Sympathetic ganglion cell loss was not prominent in the examined case.

Implications:

  • This suggests PAF can, in rare instances, be linked to preganglionic or central nervous system pathology.
