    Summary

    Giant middle cerebral artery aneurysms can mimic tumors by causing a mass effect. Surgical resection of these rare vascular lesions yielded excellent clinical outcomes in two reported cases.

    Background:

    • Giant aneurysms of the middle cerebral artery are rare vascular malformations.
    • They can present atypically, mimicking other intracranial pathologies.

    Observation:

    • Two cases of giant middle cerebral artery aneurysms presented as mass lesions.
    • Angiography demonstrated a serpentine vascular channel with surrounding avascularity, causing a mass effect.

    Findings:

    • Complete surgical resection (resection in toto) was achieved in both cases.
    • Excellent clinical results were obtained postoperatively.

    Implications:

    • This presentation highlights the importance of considering aneurysms in the differential diagnosis of intracranial mass lesions.
    • Understanding the pathophysiology of these aneurysms is crucial for diagnosis and treatment planning.
