Summary

A large calcified splenic cyst caused thrombocytopenia in a young woman. Surgical removal confirmed the cyst originated from mesothelial cells, resolving the low platelet count.

Background:

  • Splenomegaly can present with abdominal masses and cytopenias.
  • Calcified splenic lesions require thorough histopathological evaluation.

Observation:

  • A 29-year-old female presented with thrombocytopenia and a palpable abdominal mass.
  • Computed tomography revealed splenomegaly with a large, calcified cystic lesion.

Findings:

  • The spleen was surgically removed.
  • Pathological examination identified a monolocular cyst with a mesothelial origin.

Implications:

  • Mesothelial splenic cysts are rare causes of splenomegaly and thrombocytopenia.
  • Surgical excision is curative for symptomatic mesothelial splenic cysts.
