Summary

Human T-lymphotropic virus type I (HTLV-I) associated myelopathy was diagnosed in an immigrant. This case highlights the need to consider HTLV-I in unexplained myelopathy, even in low-prevalence countries.

Background:

  • Human T-lymphotropic virus type I (HTLV-I) is a retrovirus known to cause chronic neurological diseases.
  • Myelopathy is a significant neurological condition affecting the spinal cord.

Observation:

  • A 64-year-old woman, an immigrant from the Seychelles, presented with a slowly progressive myelopathy.
  • Symptoms included sphincter disturbance, back pain, and gait disturbance.
  • Laboratory tests confirmed HTLV-I infection, with no structural cause found for the myelopathy.

Findings:

  • The patient's myelopathy was attributed to HTLV-I infection.
  • Treatment with oral corticosteroids provided no benefit.
  • The virus was confirmed through serological tests (ELISA, Western blot) and molecular methods (PCR, sequencing).

Implications:

  • This case demonstrates the presence of HTLV-I associated neurological disease in Australia.
  • Clinicians should consider HTLV-I as a potential cause of myelopathy, especially in patients with relevant travel or ethnic backgrounds.
  • Further surveillance for HTLV-I infection may be warranted in specific populations within Australia.
