Malignant ameloblastoma, a rare jaw tumor, presents distinct cytologic features. However, definitively diagnosing malignancy solely through fine needle aspiration cytology can be challenging.

Area of Science:

  • Oral pathology
  • Cytopathology
  • Oncology

Background:

  • Malignant ameloblastoma is a rare odontogenic tumor originating from the jaw.
  • Accurate diagnosis is crucial for appropriate patient management and treatment planning.

Observation:

  • Fine needle aspiration (FNA) smears revealed high cellularity with basaloid and polygonal epithelial cells exhibiting mild to moderate pleomorphism.
  • Giemsa-stained smears showed a background with pinkish stroma, stellate cells, irregularly shaped nuclei, and isolated spindle cells.
  • Histopathology confirmed malignancy with features such as atypical mitosis and vascular invasion.

Findings:

  • Immunohistochemistry demonstrated positive staining for keratin in tumor cells.
  • Cytologic features were characteristic of ameloblastoma, but malignancy was difficult to ascertain definitively on FNA alone.

Related Experiment Videos

Implications:

  • FNA cytology can suggest ameloblastoma but may not be sufficient for definitive malignant diagnosis.
  • Histopathology and immunohistochemistry are essential for confirming malignant ameloblastoma.
  • Further investigation may be needed to improve diagnostic accuracy of FNA in such cases.
