Home mechanical ventilation (MV) is rare in Taiwan. This study reviewed prolonged MV patients, finding outcomes varied significantly by disorder type, with neuromuscular disorders requiring the longest duration.

Area of Science:

  • Critical Care Medicine
  • Pulmonary Medicine
  • Neurology

Context:

  • Home mechanical ventilation (MV) is uncommon in Taiwan, unlike in Western countries.
  • Prolonged MV (≥30 days, ≥8-10 hrs/day) was studied in 115 patients at Chang Gung Memorial Hospital (1990-1992).
  • Focus on patients for whom long-term MV was deemed beneficial (N=82).

Purpose:

  • To retrospectively review patient disorders and outcomes requiring prolonged mechanical ventilation.
  • To survey the home status and outcomes of twelve patients on home mechanical ventilation.
  • To analyze the association between disorder type and mechanical ventilation outcomes.

Summary:

  • Eighty-two patients requiring prolonged MV were categorized by disorder: primary pulmonary (67.07%), neuromuscular (21.95%), and brain stem (10.98%).

Related Experiment Videos

  • Outcomes included mortality (45.12%), successful weaning (25.61%), and unweanable status (hospitalized or home discharge, 29.26%).
  • Outcomes significantly correlated with disorder type (P=0.0035); neuromuscular disorders had the longest MV duration.
  • Impact:

    • Twelve patients were discharged home on MV, with a mean duration of 9.93 months.
    • Among home MV patients, four expired within two months, and two required re-admission.
    • Some patients experienced improved free time, while others remained unchanged or regressed during home care.
