[The role of two-dimensional echocardiography in the detection of pacemaker-induced endocarditis]
Unlabelled:
The authors analysed the data of eight patients who had to undergo open heart surgery because of pacemaker endocarditis in the last four years. Repeated surgical interventions on the pacemaker system was the most predisposing factor. The most common causative organisms were Staphylococcus aureus and Staphylococcus epidermidis. Two-dimensional echocardiography (2DE) had a decision making role in the diagnosis beside the clinical picture and blood culture.
Conclusions:
1. Each pacemaker patient with fever must be considered to have a pacemaker endocarditis; 2. All these patients must be examined by two-dimensional echocardiography.
Insights
Pacemaker endocarditis, an infection of the heart following pacemaker surgery, requires prompt diagnosis. Two-dimensional echocardiography is crucial for identifying this serious condition in patients with fever.
Area of Science:
- Cardiology
- Infectious Diseases
- Cardiac Surgery
Background:
- Pacemaker endocarditis is a serious complication following cardiac device implantation.
- Identifying risk factors and diagnostic methods is crucial for patient outcomes.
Observation:
- Analysis of eight patients with pacemaker endocarditis undergoing open heart surgery over four years.
- Repeated surgical interventions on pacemaker systems identified as the primary predisposing factor.
- Staphylococcus aureus and Staphylococcus epidermidis were the most frequent causative microorganisms.
Findings:
- Two-dimensional echocardiography (2DE) plays a vital role in diagnosing pacemaker endocarditis.
- Clinical presentation and blood cultures are essential but complemented by echocardiography.
- Fever in pacemaker patients necessitates consideration of endocarditis.
Implications:
- Early diagnosis and intervention are critical for managing pacemaker endocarditis.
- Two-dimensional echocardiography should be a standard diagnostic tool for febrile pacemaker patients.
- Understanding predisposing factors can guide preventative strategies in cardiac device patients.
