Summary

Aseptic loosening of the acetabular component is the most common reason for hip replacement revision surgery. Bone defects are a major challenge, often treated with allogenic bone grafts to reconstruct the acetabulum.

Area of Science:

  • Orthopaedic surgery
  • Biomaterials science

Context:

  • Over a decade, 2340 primary total hip replacements were performed.
  • 168 hip and 20 cervico-occipital prostheses underwent reoperation during the same period.

Purpose:

  • To analyze reasons for reoperation of total hip endoprostheses.
  • To evaluate the management of bone defects during revision surgery.

Summary:

  • Aseptic loosening of the acetabular component was the primary indication for hip revision (mean 98 months post-op).
  • Small acetabular components (87%) were frequently involved in loosening.
  • Bone defect reconstruction, often using allogenic grafts, is critical in acetabular revision.

Impact:

  • Highlights the challenge of bone defects in hip revision surgery.
  • Emphasizes the role of bone grafts in acetabular reconstruction.
  • Provides insights into surgical techniques for acetabular and femoral revisions.
