[Evolution of tuberculosis and lung perfusion]

    Summary

    Pulmonary tuberculosis patients with prolonged illness despite treatment showed significant lung perfusion defects on scintigraphy. These perfusion deficits, larger than visible lesions, likely hinder healing.

    Background:

    • Pulmonary tuberculosis (TB) can present with prolonged disease duration.
    • Standard antitubercular therapies may not always ensure rapid healing.

    Observation:

    • Ten patients with chronic pulmonary TB were evaluated.
    • Combined lung scintigraphy using 133Xe and 99mTc-MAA was performed.

    Findings:

    • Scintigraphy revealed perfusion defects significantly larger than radiological lesions.
    • Areas with decreased or absent lung perfusion were identified.

    Implications:

    • Reduced pulmonary perfusion may be a key factor in delayed TB healing.
    • This highlights the importance of assessing perfusion in chronic TB management.
