Summary

Ejaculatory duct obstruction, though rare, is a key cause of obstructive azoospermia. Echo-guided cyst puncture offers an effective outpatient treatment for infertility, improving sperm parameters and leading to pregnancies.

Area of Science:

  • Urology
  • Reproductive Medicine
  • Andrology

Background:

  • Ejaculatory duct obstruction is a rare but significant cause of obstructive azoospermia.
  • Diagnostic tools like seminal plasma analysis and prostatic ultrasound aid in identifying obstruction sites.
  • Surgical and endoscopic techniques have evolved for treating ejaculatory duct obstructions.

Purpose of the Study:

  • To describe the experience with treating ejaculatory duct obstructions since 1979.
  • To evaluate the efficacy of echo-guided puncture for intraprostatic cysts in infertile patients.

Main Methods:

  • 17 infertile patients with intraprostatic cysts and 2 with verumontanum stenosis were treated.
  • Azoospermic patients underwent scrototomy and endoscopy.
  • Oligozoospermic patients with severe oligozoospermia underwent echo-guided puncture of intraprostatic cysts using a 16G needle under local anesthesia.

Related Experiment Videos

  • Sclerosing agents (Sodium-tetradecyl-sulphate) were used for cysts without spermatozoa.
  • Main Results:

    • Seven out of ten subfertile patients showed increased sperm count and motility after echo-guided treatment.
    • Three patients' partners achieved pregnancy.
    • One patient required a repeat procedure due to cyst recurrence.

    Conclusions:

    • Echo-guided puncture of intraprostatic cysts is an effective, minimally invasive outpatient procedure for treating ejaculatory duct obstruction.
    • This technique can improve sperm parameters and lead to successful pregnancies in infertile men.
