Summary

Continuous negative extrathoracic pressure (CNEP) therapy for respiratory failure showed no significant impact on cardiac output in infants and young children. This pilot study found no major changes in heart rate or blood gases during CNEP treatment.

Background:

  • Continuous negative extrathoracic pressure (CNEP) is a re-emerging therapy for respiratory failure.
  • Its impact on cardiopulmonary function, particularly in pediatric populations, requires further investigation.

Purpose of the Study:

  • To evaluate the effects of CNEP on cardiac output and other hemodynamic parameters in infants and young children with chronic respiratory failure.

Main Methods:

  • A pilot study involving ten pediatric patients (2 months-3 years) with chronic respiratory failure.
  • Measurements included transcutaneous oxygen saturation, PCO2, ECG, and non-invasive pulmonary artery blood flow velocity using pulsed wave Doppler.
  • Patients were assessed both spontaneously breathing and undergoing intermittent positive pressure ventilation with and without CNEP.
