Summary

Surgical scar endometriosis, a rare condition, was treated with wide excision in 24 patients. This effective surgical approach led to no recurrences, establishing it as the preferred treatment for painful scar masses.

Background:

Surgical scar endometrioma is an uncommon condition.
Endometriosis occurring in surgical scars presents unique diagnostic and management challenges.

Purpose of the Study:

To review the clinical presentation, treatment, and outcomes of surgical scar endometrioma.
To evaluate the efficacy of wide surgical excision as the primary treatment modality.

Main Methods:

Retrospective review of 24 patients treated for surgical scar endometrioma between 1972 and 1992.
Analysis of patient demographics, prior surgical history, symptom onset, and treatment outcomes.
Follow-up evaluation of patients after wide excision.

Main Results:

The most common preceding surgery was cesarean section (19 patients).
Average time from surgery to symptom onset was 4.8 years.
All 17 patients available for follow-up after wide excision showed no recurrence.

Conclusions:

Wide surgical excision is the treatment of choice for surgical scar endometriomas.
Preoperative imaging (ultrasonography) and biopsy are recommended for asymptomatic or constantly painful masses.
Medical management is generally ineffective for this condition.
