Purpose of the Study:

  • To compare the radiographic and histological development of periapical lesions in infected teeth with open versus sealed root canals.

  • Mandibular premolars from five adult monkeys were used.
  • Teeth were infected, and root canals were either sealed or left open.
  • Periapical lesions were monitored radiographically and histologically.

Main Results:

  • Sealed infected teeth exhibited significantly earlier radiographic signs of periapical pathosis than unsealed teeth.
  • Histological signs of periapical pathology also appeared earlier in sealed teeth.
  • Unsealed teeth showed a multi-focal, diffuse pattern of histological periapical pathology.

Conclusions:

  • Sealing infected root canals accelerates the radiographic and histological manifestation of periapical lesions.
  • Root canal sealing influences the pattern of periapical tissue response to infection.
