Area of Science:

  • Oncology
  • Surgical Oncology
  • Head and Neck Cancer

Background:

  • Advanced squamous cell carcinomas of the tongue and tongue base (T3-T4) present significant management challenges.
  • Laryngeal preservation is a key consideration in optimizing functional outcomes for these patients.

Purpose of the Study:

  • To evaluate the efficacy of a combined surgical and postoperative radiotherapy approach for managing large tongue and tongue base squamous cell carcinomas.
  • To assess the feasibility of laryngeal preservation in selected patients with advanced oral cavity cancers.

Main Methods:

  • A cohort of 21 patients with T3-T4 squamous cell carcinomas of the tongue/tongue base underwent planned combined therapy.
  • Surgical management involved tumor resection with options for mandibular involvement and reconstruction using myocutaneous or muscle flaps.

Related Experiment Videos

  • All patients underwent laryngeal suspension and received postoperative megavolt radiotherapy (66 Gy in 6 weeks).
  • Main Results:

    • All patients achieved resumption of swallowing and speech.
    • Patients experienced minimal external deformity and were able to return to their families.
    • The combined approach demonstrated successful local and regional tumor control.

    Conclusions:

    • Total glossectomy with laryngeal preservation is a viable treatment option for carefully selected patients with advanced tongue and tongue base cancers.
    • This therapeutic strategy effectively achieves local and regional disease control while preserving a good quality of life.
