Area of Science:

  • Gastroenterology
  • Minimally Invasive Surgery

Context:

  • Surgical gastrostomies have been performed since the 19th century.
  • Percutaneous endoscopic gastrostomy (PEG) was introduced in 1980, revolutionizing gastric access.
  • PEG offers advantages over traditional surgical methods, including no anesthesia requirement and suitability for high-risk patients.

Purpose:

  • To evaluate the efficacy and safety of percutaneous endoscopic gastrostomy (PEG).
  • To compare PEG with surgical gastrostomy regarding risks, costs, and patient outcomes.
  • To highlight the indications and benefits of PEG for patients requiring long-term nutritional support.

Summary:

  • PEG is a minimally invasive procedure with shorter hospital stays, reduced risks, and lower costs compared to surgical gastrostomy.

  • Indications include head and neck cancers, neurological diseases, cancer cachexia, and GI tract obstructions impacting nutritional intake for over 60 days.
  • The study followed 34 patients (mean age 69) using 9-French tubes via the pull-method, with all patients tolerating enteral feeding within 3-4 days.
  • Impact:

    • PEG provides a better psychological tolerance for patients compared to nasogastric tubes.
    • The incidence of mechanical complications associated with PEG is significantly lower.
    • This technique can be performed on an outpatient basis, improving patient convenience and reducing healthcare burdens.
