Focal organizing pneumonia: CT appearance

Summary

Computed tomographic (CT) features of focal organizing pneumonia vary widely. Some lesions appear benign, but differentiating them from bronchogenic carcinoma on CT can be challenging.

Background:

Focal organizing pneumonia (OP) can mimic lung cancer on imaging.
Chest radiographs often suggest bronchogenic carcinoma in cases of focal OP.

Purpose of the Study:

To delineate the computed tomographic (CT) characteristics of focal organizing pneumonia.
To aid in differentiating focal OP from malignant lung lesions.

Main Methods:

Retrospective analysis of CT scans from 18 patients with histologically confirmed focal organizing pneumonia.
Review of chest radiographs for comparison with CT findings.

Main Results:

94% of lesions had irregular margins, 56% showed satellite lesions, and 50% had pleural tags or air bronchograms.

Three CT patterns were identified: Type A (small round mass), Type B (oval mass with pleural contact), and Type C (oval mass along bronchovascular bundle).

Certain Type B and C lesions exhibited features suggestive of benignity.

Conclusions:

CT findings in focal organizing pneumonia demonstrate considerable variability.
Distinguishing focal OP from bronchogenic carcinoma can be difficult, even with CT imaging.
Further investigation may be needed for definitive diagnosis in ambiguous cases.
