Background:

Pulmonary smooth muscle neoplasms, such as leiomyomas, are exceptionally rare findings in clinical practice.
Uterine leiomyomas are common benign tumors, and their potential for metastasis to the lung is a recognized, albeit infrequent, phenomenon.

Observation:

A 61-year-old asymptomatic woman presented with an abnormal shadow on chest X-ray, leading to surgical lung resection.
The patient had a history of hysterectomy and bilateral salpingo-oophorectomy due to uterine myoma 25 years prior.

Findings:

Histological examination confirmed the pulmonary lesion to be a leiomyoma.
No evidence of nuclear atypia or mitotic figures was observed in the pulmonary leiomyoma, indicating benignancy.
