Summary

In cardiac patients undergoing ECG, panic disorder was common and not linked to ECG abnormalities. Panic disorder patients showed higher maximal heart rates and shorter P-R intervals, possibly due to beta-adrenergic hypersensitivity.

Purpose of the Study:

  • To determine the prevalence of panic disorder in cardiac patients.
  • To investigate associations between specific abnormal ECG findings and panic disorder.

Main Methods:

  • Patients referred for ambulatory ECG were screened for anxiety using the Hospital Anxiety and Depression Scale.
  • Anxious patients (score > 8) were further assessed for panic disorder using the Schedule for Affective Disorders and Schizophrenia--Lifetime Version Modified for the Study of Anxiety Disorders (SADS-LA).

Main Results:

  • 62% of anxious cardiac patients met DSM-III-R criteria for panic disorder.
  • Panic disorder patients exhibited higher mean maximal heart rate and shorter P-R intervals compared to non-panic patients.
  • No correlation between maximal heart rate and minimal P-R interval was observed in panic disorder patients, unlike in others.

Conclusions:

  • Panic disorder is highly prevalent in patients referred for ECG.
  • The prevalence of panic disorder was similar in patients with and without ECG abnormalities, suggesting panic disorder does not exclude organic cardiac disease.
  • Elevated maximal heart rate and shorter P-R intervals in panic disorder patients may indicate beta-adrenergic receptor hypersensitivity.
