Summary

Post-rectopexy constipation is linked to impaired colonic motility. This study found significantly delayed colonic transit times and abnormal motor indexes in patients experiencing constipation after Orr rectopexy (OR).

Area of Science:

  • Gastroenterology
  • Colorectal Surgery
  • Physiology

Background:

  • Constipation following rectopexy surgery for rectal prolapse has unclear pathophysiology.
  • Potential causes include acquired anorectal dysfunction or pre-existing colonic issues.
  • Understanding colorectal motility changes post-surgery is crucial for patient management.

Purpose of the Study:

  • To prospectively investigate and characterize colorectal motility abnormalities in patients with constipation after Orr rectopexy (OR).
  • To compare motility parameters between patients post-OR, those with rectal prolapse (RP) pre-surgery, and healthy volunteers (HV).

Main Methods:

  • Prospective study of 12 patients with constipation post-OR.
  • Evaluations included symptom questionnaires, left colonic manometry, anorectal manometry, evacuation proctography, and colonic transit time.

Related Experiment Videos

  • Results compared to 10 HV and 12 RP patients.
  • Main Results:

    • Patients post-OR showed significantly prolonged global (135.9 vs 51 hr) and left colonic transit times (61.6 vs 18.2 hr) compared to RP.
    • Abnormal colonic motor activity was observed, with negative basal motor index gradients in most OR patients.
    • OR patients had lower stool frequency and higher prevalence of abdominal pain than RP patients pre-surgery.

    Conclusions:

    • Constipation after Orr rectopexy is associated with significant colonic transit delays and abnormal colonic motor function.
    • These findings suggest that colonic dysmotility plays a key role in post-rectopexy constipation.
    • Further research into managing colonic motility disorders post-rectopexy is warranted.
