Summary

Kidney stones (urolithiasis) are an uncommon complication after kidney transplants. Extracorporeal shock-wave lithotripsy successfully treated a patient with ureteropelvic junction stones without impacting kidney function.

Background:

  • Urolithiasis is an infrequently documented urologic complication following renal transplantation.
  • Understanding the management of kidney stones in transplant recipients is crucial for preserving graft function.

Observation:

  • A renal transplant patient presented with painless gross hematuria, a rare symptom in this population.
  • Intravenous pyelogram revealed a 4 x 7-mm calculus at the ureteropelvic junction, leading to moderate hydronephrosis.

Findings:

  • Successful treatment of ureteropelvic junction calculi was achieved using extracorporeal shock-wave lithotripsy (ESWL).
  • Post-treatment evaluation showed no significant changes in serum creatinine or 24-hour creatinine clearance, indicating preserved renal function.
