Background:

Peduncular hallucinosis is typically associated with structural midbrain lesions.
Understanding the etiology of peduncular hallucinosis is crucial for diagnosis and management.

Observation:

A patient presented with vivid hallucinations characteristic of peduncular hallucinosis.
Computed tomography revealed basilar arterial atherosclerosis with brain atrophy.
Carotid imaging demonstrated bilateral stenosis.

Findings:

Midbrain magnetic resonance imaging and electroencephalogram showed no abnormalities.
The patient's symptoms resolved spontaneously within 48 hours.
This presentation is proposed as the first reported case of peduncular hallucinosis secondary to transient ischemic attack (TIA) syndrome.

Implications:

This case suggests that transient ischemic attacks can manifest as peduncular hallucinosis.
Cerebrovascular disease should be considered in the differential diagnosis of peduncular hallucinosis.
Further research is warranted to explore the link between TIA and peduncular hallucinosis.
