    Diuretic withdrawal in women with "idiopathic" edema caused significant fluid retention, linked to plasma renin activity. Most cases resolved within weeks, suggesting diuretic use, not idiopathic causes, underlies this edema.

    Background:

    • Idiopathic edema is a common clinical diagnosis in women.
    • Diuretic use is prevalent, but its role in edema etiology is often overlooked.

    Purpose of the Study:

    • To investigate the link between diuretic withdrawal and edema in women diagnosed with idiopathic edema.
    • To determine the role of plasma renin activity in diuretic-induced edema.

    Main Methods:

    • Ten women with presumed idiopathic edema stopped their diuretic intake.
    • Monitored weight gain, edema, plasma renin activity, and urinary aldosterone excretion.

    Main Results:

    • All patients experienced sodium and water retention upon diuretic withdrawal.
    • Edema severity correlated with pre-withdrawal plasma renin activity.
    • Nine patients developed edema, with resolution in most within 20 days.

    Conclusions:

    • "Idiopathic" edema in women is often iatrogenic, caused by diuretic use.
    • Plasma renin activity plays a key role in the pathophysiology of diuretic-induced edema.
    • Discontinuation of diuretics can resolve edema, even in cases with prolonged symptoms.
