    Summary

    Early onset parkinsonism in two sisters was linked to reduced homovanillic acid (HVA) and serotonin metabolites in cerebrospinal fluid (CSF). A third patient with parkinsonism possibly due to chickenpox encephalitis also responded well to levodopa.

    Background:

    • Parkinsonism is a complex neurological disorder characterized by motor symptoms.
    • Early-onset parkinsonism presents before age 50 and can have diverse etiologies.
    • Cerebrospinal fluid (CSF) analysis offers insights into neurotransmitter levels.

    Observation:

    • Three patients presented with early-onset parkinsonism.
    • Two patients, sisters, exhibited reduced levels of homovanillic acid (HVA), 5-hydroxytryptamine (5-HT), and 5-hydroxyindole acetic acid (5-HIAA) in CSF.
    • The third patient's parkinsonism was potentially linked to chickenpox encephalitis.

    Findings:

    • Reduced CSF levels of dopamine metabolite HVA and serotonin metabolites 5-HT and 5-HIAA were observed in two siblings with early-onset parkinsonism.
    • Chickenpox encephalitis was a potential cause of parkinsonism in the third patient.
    • All three patients demonstrated a positive clinical response to levodopa treatment.

    Implications:

    • This case series suggests a potential biochemical link (reduced HVA, 5-HT, 5-HIAA) in some early-onset parkinsonism cases.
    • Levodopa therapy is effective for managing parkinsonism symptoms, regardless of suspected etiology in these cases.
    • Further research into the specific mechanisms underlying neurotransmitter reduction in early-onset parkinsonism is warranted.
