    Summary

Conjunctivitis of Morax, a rare condition caused by beta-hemolytic streptococci, can lead to severe corneal ulcers. Prompt antibiotic treatment is crucial for recovery from this hyperacute bacterial conjunctivitis.

    Area of Science:

    • Ophthalmology
    • Microbiology

    Background:

    • Nasolacrimal duct obstruction can predispose individuals to secondary infections.
    • Beta-hemolytic streptococci are a known cause of bacterial conjunctivitis.

    Observation:

    • A 72-year-old woman presented with hyperacute conjunctivitis and a marginal furrow-like corneal ulcer.
    • The patient had a pre-existing low-grade nasolacrimal duct obstruction.
    • A plastic iritis was also noted concurrently with the corneal ulcer.

    Findings:

    • The conjunctivitis was diagnosed as "Conjunctivitis of Morax" and was caused by beta-hemolytic streptococci.
    • A corneal ulcer, likely a stromal infection, developed due to conjunctival chemosis trapping exudate.
    • Treatment with topical erythromycin, lacrimal sac irrigation with penicillin, and oral penicillin led to improvement.

    Implications:

    • Conjunctivitis of Morax requires prompt diagnosis and treatment to prevent corneal complications.
    • Understanding the pathogenesis of corneal ulceration in this condition is important for ophthalmologists.
    • Clinical differentiation from other severe conjunctivitis forms, such as pneumococcal dacryocystitis or Neisseria infections, is essential.
