Peripheral iridectomy in closed angle glaucoma-- late complications

N Floman, D Berson, L Landau

    The British Journal of Ophthalmology

    Summary

Peripheral iridectomy (PI) carries risks like vision loss and complications. Routine PI on the second eye is not advised unless specific glaucoma indicators are present.

    Area of Science:

    • Ophthalmology
    • Surgical Procedures
    • Glaucoma Management

    Background:

    • Peripheral iridectomy (PI) is a surgical procedure used in glaucoma management.
    • The risks and benefits of PI, particularly in prophylactic settings, require careful evaluation.

    Purpose of the Study:

    • To assess the incidence of complications following therapeutic and prophylactic peripheral iridectomy (PI).
    • To determine if prophylactic PI on the second eye is warranted without acute attack or specific clinical signs.

    Main Methods:

    • Retrospective analysis of 70 eyes undergoing therapeutic PI.
    • Evaluation of complications in eyes undergoing prophylactic PI.
    • Assessment of visual acuity, posterior synechiae (PS), and lens opacities.

    Main Results:

    • Therapeutic PI: 51% vision loss (≥2 lines), 57% PS, 57% lens opacities.
    • Prophylactic PI: 33% vision loss (≥2 lines), 39% PS, 42% lens opacities.
    • Complications were less frequent but still significant in the prophylactic group.

    Conclusions:

    • Peripheral iridectomy (PI) is associated with notable risks, including vision impairment and ocular complications.
    • Routine prophylactic PI on the second eye is not recommended.
    • PI should be reserved for cases with confirmed angle-closure glaucoma or positive provocative tests.
