Pain Management in Pediatrics: What the IR has to Offer.

Background:

  • Hepatic undifferentiated mesenchymal sarcoma is a rare pediatric malignant neoplasm.
  • Early diagnosis and accurate staging are crucial for treatment planning.

Observation:

  • Three pediatric cases (ages 7, 8, 12) presented with abdominal pain, mass, asthenia, anorexia, and weight loss.
  • Jaundice was noted in one patient.
  • Ultrasound (US), computed tomography (CT), and magnetic resonance imaging (MRI) detected the lesions.

Findings:

  • MRI provided more accurate lesion localization and resectability assessment compared to US and CT.
  • Characteristic MRI findings included marked hyperintensity on long TR/TE spin-echo (SE) and short-time inversion recovery (STIR) sequences due to cystic areas, myxoid material, and necrosis.
  • Internal separations were hypointense, fibrous pseudocapsules were seen in two cases, and hemorrhage was hyperintense on short TR/TE SE in two cases.
  • MRI also identified vascular invasion, biliary obstruction, and hilar adenopathies.

Implications:

  • The combination of hemorrhage and cystic/myxoid components on MRI is a common feature of this tumor.
  • MRI's detailed characterization aids in surgical planning and prognosis for pediatric hepatic undifferentiated mesenchymal sarcoma.
  • Advanced imaging techniques like MRI are vital for diagnosing and managing rare pediatric liver malignancies.
