Summary

Fluid analysis in osteoarthritis (OA) and total hip replacement (THR) pseudojoints reveals differences in hyaluronate. THR arthroplasties appear to stimulate hyaluronate synthesis by lining cells.

Background:

  • Osteoarthritis (OA) affects hip joint fluid composition.
  • Total hip replacement (THR) surgery creates a pseudojoint with distinct fluid characteristics.

Purpose of the Study:

  • To compare the protein and hyaluronate concentrations and molecular weight in hip OA joint fluid versus THR pseudojoint fluid.
  • To investigate the adaptive tissue response in THR pseudojoints.

Main Methods:

  • Analysis of synovial fluid samples from primary hip OA (N=21) and THR revision for loosening (N=17).
  • Measurement of protein concentration.
  • Quantification and molecular weight determination of hyaluronate using high-performance liquid chromatography (HPLC) and laser light scatter analysis.

Main Results:

  • Protein concentrations were similar in both OA and THR pseudojoint fluid.
  • Hyaluronate concentration was significantly higher in OA fluid compared to THR pseudojoint fluid.
  • OA synovial fluid hyaluronate had a lower molecular weight than THR pseudojoint fluid.

Conclusions:

  • Similar protein levels suggest comparable transcapillary flow in both joint types.
  • THR arthroplasties induce an adaptive tissue response, forming fibroblast-like cells that synthesize hyaluronate.
  • The observed differences in hyaluronate suggest distinct biological processes in native OA joints versus THR pseudojoints.
