Summary

Patients with rosacea have significantly higher Demodex mite populations on their facial skin compared to healthy individuals. Tetracycline treatment did not reduce Demodex mite density in rosacea patients.

Background:

  • The etiology of rosacea remains unclear, with Demodex mites (Demodex folliculorum and Demodex brevis) being investigated as potential causative agents.
  • Research exploring the link between Demodex mites and rosacea pathogenesis is ongoing.

Purpose of the Study:

  • To quantitatively compare Demodex mite populations in facial skin of rosacea patients versus control subjects.
  • To evaluate the effect of oral tetracycline therapy on Demodex mite density in patients with rosacea.

Main Methods:

  • A comparative study involving 42 patients diagnosed with rosacea and 42 age- and sex-matched healthy controls.
  • Demodex mite population density was assessed using skin surface biopsy specimens collected with cyanoacrylate glue from six standard facial sites.

Main Results:

  • Patients with rosacea exhibited significantly higher mean Demodex mite counts (49.8) than control subjects (10.8) (p < 0.001).
  • Elevated mite density was observed across all rosacea subtypes, most notably in steroid-induced rosacea, with the cheeks showing the highest concentration.
  • A 1-month course of oral tetracycline did not result in a significant reduction of Demodex mite counts in rosacea patients.

Conclusions:

  • Increased Demodex mite populations are strongly associated with rosacea.
  • Demodex mites may contribute to rosacea pathogenesis through inflammatory responses, follicular blockage, or by acting as microbial vectors.
  • Current tetracycline regimens may not effectively reduce Demodex mite burden in rosacea patients.
