Background:

  • Carcinosarcoma of the urinary bladder is an exceptionally rare malignancy.
  • This tumor type presents with a poor prognosis and is often diagnosed at an advanced stage.

Observation:

  • A case of a large urinary bladder carcinosarcoma is presented, diagnosed 2 months prior to treatment.
  • Initial transurethral resection was followed by radical cystectomy.
  • The patient experienced local recurrence and widespread metastasis 7 months post-surgery, leading to death.

Findings:

  • Superficial carcinosarcoma of the bladder invariably invades the lamina propria.
  • Sarcomatous elements infiltrate the submucosa, even when the overlying mucosa appears intact.
  • Local surgical interventions like transurethral resection (TUR) or partial cystectomy carry a significant risk of incomplete tumor removal.

Implications:

  • Radical cystectomy is indicated as the preferred treatment for both superficial and invasive urinary bladder carcinosarcomas.
  • Early diagnosis and aggressive surgical management are crucial for improving outcomes in these rare tumors.
