    Summary

    Severe psoriasis patients undergoing PUVA therapy sometimes develop antinuclear antibodies (ANA). These antibodies did not indicate collagenosis and did not impede successful PUVA treatment outcomes.

    Background:

    • Psoriasis is a chronic autoimmune skin condition.
    • PUVA (psoralen plus ultraviolet A) therapy is a common treatment for severe psoriasis.
    • The potential immunological effects of PUVA therapy are not fully understood.

    Purpose of the Study:

    • To investigate the incidence of antinuclear antibodies (ANA) in patients with severe psoriasis undergoing PUVA therapy.
    • To determine if ANA development correlates with collagenosis or affects treatment response.

    Main Methods:

    • Prospective study of 34 patients with severe psoriasis receiving PUVA therapy.
    • Pre-treatment and during-treatment screening for antinuclear antibodies (ANA).
    • Specific testing for antibodies against native DNA and screening for collagenosis.

    Main Results:

    • Seven out of 34 patients (21%) developed circulating ANA during PUVA therapy.
    • Pre-treatment ANA was detected in only 3 out of 50 patients (6%) considered for PUVA.
    • ANA titres were generally low, and antibodies against native DNA were not detected.
    • No evidence of collagenosis was found in patients developing ANA.
    • All 7 patients with newly developed ANA responded well to PUVA treatment.

    Conclusions:

    • PUVA therapy may induce the development of circulating antinuclear antibodies (ANA) in a subset of patients with severe psoriasis.
    • The development of ANA during PUVA therapy does not appear to be associated with collagenosis.
    • Newly developed ANA in this context do not negatively impact the efficacy of PUVA treatment for psoriasis.
