Summary

Hot perineal baths do not relax anal sphincters in healthy individuals. This study found no significant changes in anal pressure before or after a hot bath, questioning its effectiveness for anorectal conditions.

Area of Science:

  • Gastroenterology and Surgical Research
  • Physiology of the Anorectal Region

Background:

  • Hot perineal baths are commonly recommended for painful anorectal conditions like anal fissures and perianal hematomas.
  • They are also frequently used for post-hemorrhoidectomy care.
  • However, the physiological basis for their efficacy has not been scientifically investigated.

Purpose of the Study:

  • To investigate the physiological effects of hot perineal baths on anal sphincter pressure.
  • To determine if hot baths induce anal sphincter relaxation in subjects without anorectal complaints.

Main Methods:

  • Anorectal manometry was utilized to measure anal pressures.
  • Measurements were taken from 40 healthy control subjects before and after a hot perineal bath.
  • Resting and voluntary contraction anal pressures were assessed.

Related Experiment Videos

Main Results:

  • No statistically significant difference was observed in resting anal pressures before and after the hot perineal bath.
  • No significant difference was found in anal pressures during voluntary contraction.
  • The procedure did not lead to measurable relaxation of the anal sphincters in normal subjects.

Conclusions:

  • Hot perineal baths do not appear to cause relaxation of the anal sphincters in individuals without anorectal pathology.
  • The study suggests a lack of physiological rationale for the widespread use of hot baths for certain anorectal conditions.
  • Further research may be needed to explore potential benefits or alternative explanations for symptom relief.
