Summary

Primary breast lymphoma is rare and presents differently in younger versus older women. This study found that older women’s tumors were typically high-grade B-cell lymphomas, not associated with mucosa-associated lymphoid tissue.

Background:

  • Primary breast lymphoma is an uncommon malignancy with diverse clinicopathologic presentations.
  • Literature suggests two main groups: one in young women (Burkitt-type) and another in older women (B-cell non-Hodgkin's type) mimicking breast carcinoma.
  • Recent research indicates a subset of lymphomas in older women may originate from mucosa-associated lymphoid tissue (MALT).

Purpose of the Study:

  • To review and characterize nine cases of primary breast lymphoma diagnosed between 1974 and 1990.
  • To evaluate the clinicopathologic and immunohistochemical features of these tumors.
  • To screen for t[14;18] translocation in relevant cases.

Main Methods:

  • Retrospective review of clinical features for nine primary breast lymphoma cases.

    • All nine cases occurred in women over 50 presenting with symptoms similar to breast carcinoma.
    • Eight cases were B-cell lymphomas, with seven classified as high-grade; one was a true follicular lymphoma.
    • One case was a true histiocytic lymphoma. None of the cases exhibited features of MALT lymphoma.

    Conclusions:

    • Primary breast lymphomas in older women in this cohort were predominantly high-grade B-cell lymphomas.
    • These tumors presented clinically like breast carcinoma.
    • The findings did not support the presence of MALT lymphomas within this specific patient group.
