Summary

Functional communication training (FCT) alone was insufficient for some patients with severe behavior problems. Combining FCT with punishment yielded the most significant behavior reductions.

Area of Science:

  • Behavioral Science
  • Clinical Psychology
  • Developmental Disabilities

Background:

  • Severe behavior problems in individuals with intellectual disabilities pose significant challenges.
  • Functional communication training (FCT) is a promising intervention for addressing these issues.
  • Existing research suggests FCT may need augmentation for certain patient populations.

Purpose of the Study:

  • To evaluate the efficacy of FCT alone versus FCT combined with extinction and/or punishment.
  • To assess treatment outcomes for individuals with severe intellectual disabilities, behavior problems, and communication deficits.
  • To determine the most effective intervention strategy for reducing severe destructive behaviors.

Main Methods:

  • Four inpatient clients with severe intellectual disabilities and behavior disorders participated.

Related Experiment Videos

  • Individualized interventions were based on functional assessments.
  • Reinforcement was provided for communication responses that served the same function as destructive behaviors.
  • Main Results:

    • Functional communication training alone was not sufficient for all patients to achieve clinically significant behavior reduction.
    • The combination of FCT with punishment resulted in the largest and most consistent reductions in destructive behavior.
    • Individualized interventions based on functional assessment were employed.

    Conclusions:

    • While FCT is a valuable tool, it may require adjuncts like punishment for some individuals with severe behavior disorders.
    • Combined interventions, particularly FCT with punishment, appear most effective for reducing severe destructive behaviors in this population.
    • Functional assessment-based interventions are critical for tailoring effective treatment plans.
