Tests for completeness of vagotomy

M G Moshal, J M Spitaels, G Dimopoulos

South African Medical Journal = Suid-Afrikaanse Tydskrif Vir Geneeskunde
    |February 26, 1977

    Summary

Post-vagotomy patients with recurrent ulcers showed higher acid concentration after insulin and lower serum gastrin. Basal acid output alone provided significant diagnostic information for recurrent duodenal ulcers.

    Area of Science:

    • Gastroenterology
    • Surgical Outcomes
    • Acid Secretion Studies

    Background:

    • Truncal vagotomy is a surgical procedure to reduce gastric acid production.
    • Recurrent duodenal ulcers can occur post-vagotomy, necessitating effective diagnostic methods.
    • Assessing post-surgical gastric function is crucial for patient management.

    Purpose of the Study:

    • To identify diagnostic tests differentiating patients with recurrent duodenal ulcers after truncal vagotomy from those without.
    • To evaluate the efficacy of various gastric acid secretion and serum gastrin tests.

    Main Methods:

    • A cohort of 60 patients, 2-24 months post-truncal vagotomy (with pyloroplasty or gastro-enterostomy), underwent a battery of tests.
    • Comparison between patients with and without endoscopic evidence of recurrent duodenal ulcers.

    Related Experiment Videos

  • Analysis included insulin and pentagastrin stimulation tests, basal acid output, and serum gastrin levels.
  • Main Results:

    • Increased first-hour acid concentration post-insulin and lower serum gastrin levels significantly differentiated patients with recurrent ulcers.
    • Postoperative decrease in basal and maximal acid output were useful but not statistically significant indicators.
    • Symptoms and combined stimulation tests showed poor differentiation efficacy.

    Conclusions:

    • Serum gastrin and post-insulin acid concentration are key indicators for diagnosing recurrent ulcers after vagotomy.
    • Postoperative basal acid output offers substantial diagnostic value, comparable to complex stimulation tests.
    • Further research into optimizing diagnostic markers for post-vagotomy complications is warranted.
