Fluconazole

Area of Science:

  • Pharmacology
  • Transplantation Medicine

Background:

  • Cyclosporine is a critical immunosuppressant post-transplant.
  • Fluconazole is a common antifungal agent.
  • Drug interactions can impact patient outcomes.

Observation:

  • A renal-pancreatic transplant patient experienced increased cyclosporine levels and serum creatinine with fluconazole 300 mg/d.
  • No significant changes were observed at fluconazole 100 mg/d.
  • Bone marrow transplant recipients on fluconazole 100 mg/d showed no significant cyclosporine alterations.

Findings:

  • The interaction between fluconazole and cyclosporine appears to be dose-dependent.
  • Higher fluconazole doses (300 mg/d) significantly increased cyclosporine levels and nephrotoxicity markers.
  • Lower fluconazole doses (100 mg/d) did not demonstrate this interaction.

Related Experiment Videos

Implications:

  • Clinicians should consider fluconazole's dose-dependent effect on cyclosporine metabolism.
  • Monitoring cyclosporine levels and renal function is crucial when using fluconazole >200 mg/d.
  • Dosage adjustments of cyclosporine may be necessary to prevent toxicity.
