Incidental epidermolytic hyperkeratosis in progressive systemic sclerosis

Summary

This study reports incidental epidermolytic hyperkeratosis (EH) in a patient with progressive systemic sclerosis (PSS). This rare finding was not observed in other PSS or scleroderma cases reviewed.

Background:

Epidermolytic hyperkeratosis (EH) is a recognized histopathological finding in various skin lesions.
Progressive systemic sclerosis (PSS) is an autoimmune connective tissue disease characterized by skin thickening.

Observation:

The authors describe a case of incidental EH in a patient diagnosed with PSS.
A review of 108 skin biopsies from PSS and localized scleroderma cases revealed EH only in this specific patient.

Findings:

The observed histopathological pattern was deemed most consistent with EH, despite its rarity in PSS.
Focal acantholytic dyskeratosis, a similar condition, was considered but deemed less fitting than EH.

Implications:

This case highlights a potentially new, albeit rare, association between EH and PSS.
