Background:

  • Nephroptosis, or kidney prolapse, can cause severe pain and complications like hydronephrosis.
  • Traditional surgical correction for nephroptosis can be invasive with significant recovery times.

Observation:

  • A 25-year-old female presented with chronic intractable pain and a palpable pelvic mass due to right nephroptosis.
  • Intravenous urography confirmed significant right renal descent (three vertebral bodies) and mild hydronephrosis.

Findings:

  • Laparoscopic transperitoneal nephropexy was successfully performed, taking 2.5 hours.
  • Postoperatively, the patient experienced rapid recovery, returning to full activities in 3 weeks.
  • At 2 months, the patient was asymptomatic with improved renal position (one vertebral body descent) and resolved hydronephrosis.

Implications:

  • Laparoscopic nephropexy is a viable, minimally invasive option for treating symptomatic nephroptosis.
  • This technique offers reduced recovery time and effective symptom resolution compared to open surgery.
  • This case establishes a precedent for successful laparoscopic management of kidney prolapse.
