Tonometry screening on the medical service

D Robertson

    Archives of Internal Medicine

    Summary

Routine Schiotz tonometry screening for glaucoma in general medical patients over 45 years old identified a 3.3% prevalence. This study supports its utility for internists due to high glaucoma prevalence and low risk.

    Area of Science:

    • Ophthalmology
    • Internal Medicine
    • Public Health

    Background:

    • Glaucoma is a leading cause of irreversible blindness.
    • Early detection through screening is crucial for managing glaucoma.
    • General medical services encounter a significant population at risk for glaucoma.

    Purpose of the Study:

    • To evaluate the effectiveness of Schiotz tonometry for glaucoma screening in a general medical inpatient setting.
    • To determine the prevalence of glaucoma in patients over 45 years old admitted to a general medical service.

    Main Methods:

    • Schiotz tonometry was performed on patients over 45 years old over a two-year period.
    • Patients with intraocular pressure (IOP) ≥ 21 mm Hg were referred for ophthalmologic evaluation.
    • Prevalence of diagnosed glaucoma or suspected glaucoma was calculated.

    Main Results:

    • 61% of patients were over 45 years old.
    • 5.5% of this group had IOP ≥ 21 mm Hg and were referred.
    • 44% of referred patients were diagnosed with glaucoma or suspected glaucoma, yielding an overall prevalence of 3.3%.

    Conclusions:

    • Schiotz tonometry is a valuable, low-risk screening tool for internists to detect glaucoma in older patients.
    • The high prevalence of glaucoma in the surveyed population underscores the importance of routine screening.
    • Integrating tonometry into general medical services can improve early glaucoma detection rates.
