    Frozen-thawed red blood cells (RBCs) offer a transfusion alternative for paroxysmal nocturnal hemoglobinuria (PNH) patients. This approach reduces complement-sensitive cells, mitigating hemolysis during major vascular surgery.

    • Standard treatment involves saline-washed red blood cells (RBCs).

    Observation:

    • Frozen-thawed, deglycerolized RBCs can substitute for saline-washed RBCs in PNH patients.
    • Transfusion of these RBCs decreases bone marrow production of RBCs.
    • This leads to a reduced percentage of complement-sensitive erythrocytes.

    • A decreased percentage of complement-sensitive erythrocytes allows major vascular surgery without hemolytic complications.
