Summary

Extraskeletal osteosarcoma of the thigh is rare and may not show calcifications on MRI. Imaging findings can be non-specific, but this diagnosis should be considered for thigh soft-tissue masses.

Background:

  • Extraskeletal osteosarcoma is a rare malignant neoplasm.

Purpose of the Study:

  • To illustrate the magnetic resonance imaging (MRI) findings of extraskeletal osteosarcoma of the thigh.
  • To correlate imaging findings with pathological examination in four cases.

Main Methods:

  • Retrospective review of four cases of extraskeletal osteosarcoma of the thigh.
  • Analysis of MRI findings, including T1-weighted and T2-weighted imaging, with and without contrast administration.
  • Radiologic-pathologic correlation.

Main Results:

  • Calcifications or osteoid material were not visible on MRI or conventional radiography in any of the four tumors.

  • Three tumors appeared well-demarcated on MRI, correlating with pseudoencapsulation.
  • These three tumors showed heterogeneous appearance, hyperintensity to muscle on T1-weighted imaging, and high signal intensity on T2-weighted imaging.
  • The fourth tumor exhibited cystic, hemorrhagic, and solid components on MRI, confirmed by pathology.
  • Conclusions:

    • MRI findings in extraskeletal osteosarcoma are non-specific.
    • Extraskeletal osteosarcoma should be included in the differential diagnosis of thigh soft-tissue masses, especially in the appropriate age group, even without detectable calcifications.
