Background:

  • Prostatic carcinosarcoma is an exceptionally rare malignant neoplasm.
  • This case represents the youngest patient diagnosed with this condition.

Observation:

  • A 32-year-old male presented with prostatic carcinosarcoma.
  • The resected tumor exhibited both undifferentiated carcinoma and diverse mesodermal sarcoma components.
  • Histological examination revealed myxoid areas, spindle cell sarcoma with nuclear palisading, osteoid formation, and chondromatous differentiation.

Findings:

  • The patient underwent total prostatectomy, followed by adjuvant chemotherapy and radiotherapy.
  • Despite aggressive treatment, the patient developed widespread lung metastases and died approximately 8 months post-surgery.
  • The tumor demonstrated intimate intermingling of carcinomatous and sarcomatous elements with transitional forms.

Implications:

  • The case underscores the aggressive clinical behavior and poor prognosis associated with prostatic carcinosarcoma.
  • The unique histology suggests a potential origin from vestigial embryologic Müllerian mesodermal tissue with multipotent differentiation capabilities.
  • Further research into the pathogenesis and molecular mechanisms of prostatic carcinosarcoma is warranted.
