    Purpose of the Study:

    • To assess the long-term effects of cyclical chemotherapy on fertility and gonadal function in male patients with advanced Hodgkin's disease.
    • To evaluate the recovery of spermatogenesis and hormonal profiles post-chemotherapy.

    Main Methods:

    • Seventy-four male patients with advanced Hodgkin's disease were studied.
    • Assessment included azoospermia evaluation, testicular biopsy, hormonal assays (FSH, LH, testosterone, prolactin), and follow-up for spermatogenesis recovery.
    • Libido, sexual performance, and contraceptive practices were also recorded.

    Main Results:

    • All patients were azoospermic post-therapy; only 4/74 regained spermatogenesis after a median of 27 months.
    • Testicular biopsies revealed absence of germinal epithelium.
    • Follicle-stimulating hormone (FSH) levels remained elevated, luteinizing hormone (LH) levels were borderline high, and testosterone levels were normal. Prolactin levels increased in 42% of patients.
    • Spermatogenesis recovery could not be predicted by hormone levels.

    Conclusions:

    • Cyclical chemotherapy leads to high rates of persistent infertility in male Hodgkin's disease survivors.
    • Sperm storage before treatment is crucial due to low and unpredictable recovery rates.
    • Discussion of potential sexual dysfunction and provision of contraceptive advice are important for patient management.
