Summary

Obstructive sleep apnea (OSA) can cause myocardial ischemia, indicated by ST depression. Nasal continuous positive airway pressure (CPAP) significantly reduced ST depression duration in OSA patients.

Background:

  • Obstructive sleep apnea (OSA) is linked to cardiovascular complications.
  • Myocardial ischemia, indicated by ST-segment depression, may occur during sleep in OSA patients.

Purpose of the Study:

  • To investigate the occurrence of ST depression during sleep in patients with OSA.
  • To evaluate the effect of nasal continuous positive airway pressure (CPAP) on ST depression in these patients.

Main Methods:

  • Overnight sleep studies and Holter monitoring were conducted on 23 OSA patients.
  • Patients received nasal CPAP for half the night, with random assignment.
  • ST depression was defined as >1 mm from baseline for >1 minute.

  • Seven patients (30%) experienced ST depression during sleep.
  • Nasal CPAP reduced ST depression duration (30 vs. 11 min/hr sleep).
  • CPAP also decreased apnea-hypopnea index, arousal index, and time with oxygen saturation <90%.

  • ST depression during sleep is common in OSA patients.
  • Nasal CPAP therapy significantly reduces the duration of ST depression.
  • OSA may precipitate myocardial ischemia, responsive to CPAP treatment.
