Summary

Elderly individuals with exocrine pancreatic insufficiency show a diminished plasma pancreatic polypeptide response to meals. A less than 100% increase in pancreatic polypeptide levels after eating indicates this condition.

Purpose of the Study:

  • To investigate the relationship between exocrine pancreatic function and plasma pancreatic polypeptide (PP) levels in elderly subjects.
  • To determine if plasma PP levels can serve as an indicator for exocrine pancreatic insufficiency in the elderly.

Main Methods:

  • Studied 14 normal elderly subjects and 10 elderly patients with diagnosed EPI using the para-aminobenzoic acid (PABA) test.
  • Administered a standard mixed meal to all participants.

    • Patients with EPI exhibited a significantly decreased total PP response after a mixed meal compared to controls (t = 2.753, p = 0.01).
    • An increase in plasma PP levels of less than 100% from basal 30 minutes post-meal was strongly associated with EPI (Fisher's exact test, p = 0.005).

    Conclusions:

    • Reduced plasma pancreatic polypeptide response after a meal is a reliable indicator of exocrine pancreatic insufficiency in the elderly.
    • Plasma PP level monitoring offers a potential non-invasive method for identifying EPI in geriatric populations.
