Background:

  • Strabismus (eye misalignment) is a frequent complication in individuals with spina bifida or meningomyelocele.
  • The visual system in these patients can be affected by hydrocephalus, potentially leading to acquired defects.

Purpose of the Study:

  • To investigate the prevalence and characteristics of strabismus in patients with spina bifida.
  • To explore the correlation between A-pattern strabismus and neuroimaging findings in the brainstem.

Main Methods:

  • Retrospective review of 1544 eye examinations from 298 patients with spina bifida over 15 years.
  • Analysis of visual acuity, binocular function, and strabismus prevalence.
  • Neuroimaging in select patients with A-pattern strabismus.

Main Results:

  • 85% of patients had correctable visual acuity to 20/40.
  • 61% of patients experienced strabismus during the study period, with esodeviation (63%) more common than exodeviation (37%).

Conclusions:

  • Given increased longevity, monitoring and treating visual system defects, including strabismus and amblyopia, is crucial for the quality of life in spina bifida patients.
  • Neuroimaging can reveal correlations between brainstem abnormalities and specific strabismus patterns.
