Background:

Sarcoidosis is an inflammatory disease that can affect multiple organs, commonly presenting in young adults.
While lung and skin manifestations are frequent, ocular and renal involvement are less common but significant.
Ocular symptoms occur in up to 25% of patients, whereas renal disease is rarer.

Observation:

A young male patient presented with uveitis, a common ocular manifestation of sarcoidosis.
Shortly after the ocular symptoms, the patient developed acute renal failure.
This combination of uveitis and renal failure is an uncommon presentation of sarcoidosis.

Findings:

The patient's renal failure was diagnosed as being caused by sarcoidosis.
The specific renal pathology included granulomatous interstitial nephritis and hypercalcaemic nephropathy.
This case illustrates a rare co-occurrence of ocular and renal sarcoidosis.

Implications:

This case underscores the need for heightened awareness of potential renal complications in patients presenting with ocular sarcoidosis.
Early recognition and management of renal involvement in sarcoidosis are crucial to prevent irreversible kidney damage.
The unusual combination of systemic features presented may aid in diagnosing complex sarcoidosis cases that mimic other vasculitides.
