Summary

Herpes zoster ophthalmicus (HZO) can rarely cause delayed chorioretinopathy, characterized by retinal pigment epithelial and choroidal atrophy. This condition, termed herpes zoster chorioretinopathy, may stem from an occlusive vasculitic process.

Background:

  • Herpes zoster ophthalmicus (HZO) is a viral infection affecting the ophthalmic nerve.
  • Chorioretinitis and pigment atrophy are rare but serious complications following HZO.

Observation:

  • Two patients developed posterior fundus abnormalities months after acute HZO episodes.
  • These abnormalities included yellow, non-pigmented areas of retinal pigment epithelial and choroidal atrophy.

Findings:

  • The observed condition was termed herpes zoster chorioretinopathy.
  • A proposed pathogenesis involves an occlusive vasculitic process, potentially similar to delayed cerebral vasculitis post-HZO.

Implications:

  • Clinicians should inquire about prior HZO history in patients presenting with unilateral, multifocal, non-pigmented chorioretinopathy.
  • Herpes zoster chorioretinopathy may be a characteristic delayed sequela of HZO.
