Summary

Renal trauma management in emergency settings shows conservative approaches are effective. Most patients with renal trauma can be managed non-operatively, with low morbidity and mortality rates observed in the abstention group.

Area of Science:

  • Urology
  • Trauma Surgery
  • Emergency Medicine

Background:

  • Renal trauma represents a significant challenge in emergency medicine.
  • Understanding injury mechanisms and diagnostic modalities is crucial for effective management.

Purpose of the Study:

  • To analyze the characteristics, management, and outcomes of patients with renal trauma.
  • To evaluate the efficacy of conservative versus surgical management strategies.

Main Methods:

  • Retrospective analysis of 65 renal trauma patients admitted between September 1989 and September 1994.
  • Assessment of injury mechanisms, hematuria presence, diagnostic imaging (echography), and treatment approaches (surgical vs. non-operative).

Main Results:

Related Experiment Videos

  • Contusions and deceleration injuries were the most common mechanisms.
  • Echography demonstrated high sensitivity for perirenal hematomas (100%) but lower for parenchymal lesions (65%).
  • Non-operative management was successful in most cases, with lower specific morbidity (10.7%) compared to surgical intervention (20%).
  • Conclusions:

    • Conservative management is a safe and effective strategy for the majority of renal trauma cases.
    • Early surgical intervention is reserved for hemodynamically unstable patients.
    • Systematic abdominal echography is a valuable tool in diagnosing renal trauma.
