  • Diagnosis confirmed via aspiration biopsy; patient declined surgical intervention.
  • Tumor size and invasion remained stable over 42 months of follow-up.

  • This case adds to the limited literature, with only 20 reported instances of bilateral synchronous renal oncocytoma globally.
  • Multifocal lesions were present in only 6 of the reported bilateral cases, underscoring the rarity.
